What is Complete Heart Failure Care?

Heart failure happens when the heart becomes too weak or stiff to pump blood as effectively as it should. At Current Clinic, we take a coordinated approach to improve your symptoms, protect your heart, and get the support you need from diagnosis to ongoing care.

Our approach focuses on four key areas of care

Complete Heart Failure Care is tailored to you and designed to improve heart function, reduce symptoms, and lower your risk of complications.

Advanced diagnostic testing

The first step in treating heart failure is understanding what’s causing it and how it’s affecting your heart. We use advanced diagnostic testing to evaluate your heart function and identify the underlying cause. This helps determine the most effective treatment and ensures your care is tailored to your specific needs.

Diagnosing heart failure:

Echocardiogram

An ultrasound of the heart that shows its structure and how well it pumps.

Stress testing

A stress test evaluates how the heart performs during physical activity or simulated stress to identify problems that may not appear at rest.

Cardiac catheterization

A minimally invasive imaging procedure that allows doctors to examine the heart and arteries to check for blockages or other underlying issues.

Guideline-directed medical therapy (GDMT)

Although there is no cure for heart failure, several medications have been shown to help people with heart failure live longer and feel better. These medications work best when strategically adjusted over time to reach the most effective dose, something that requires consistent follow-up and support.

At Current Clinic, we don’t just give you a prescription — we work closely with you over time to adjust your treatment based on how your body responds. This process ensures you get the right doses of the right medications at every stage of your treatment journey.

GDMT medications:

ACE inhibitors, ARBs, or ARNIs

Medications that relax blood vessels, lower blood pressure, and reduce stress on the heart.

Beta blockers

Beta blockers slow your heart rate and lower blood pressure, helping your heart work more efficiently.

Mineralocorticoid receptor antagonists (MRAs)

MRAs help the body remove excess salt and fluid while also protecting the heart from further damage.

SGLT2 inhibitors

SGLT2 inhibitors help improve heart function and have been shown to reduce hospitalizations and improve outcomes in people with heart failure.

Team-based care

Heart-failure care often requires more than one type of specialist. At Current Clinic, your care is delivered by a team of experts who work together to manage every aspect of your treatment, regularly reviewing your progress, sharing insights, and making decisions together based on the full picture of your health.

Components of team-based care:

Your care is guided by a team of providers with expertise across the full spectrum of heart-failure management.

Your care team works together to review your progress and help you make decisions about your treatment.

We collaborate with additional specialists when more specialized care is needed.

Your care plan is reviewed regularly so we can catch changes early and adjust your treatment as needed.

Care coordination and ongoing support

Managing heart failure can be complex — but getting care shouldn’t be. That’s why our team handles the details behind the scenes. From scheduling appointments and follow-ups to communicating with other providers, we make it easy to get the support you need — so you can focus on your health, not the logistics.

Components of care coordination and ongoing support:

Help scheduling appointments, follow-ups, and coordinating care with your providers.

A dedicated point of contact for check-ins, questions, and guidance throughout your care.

At-home monitoring tools (such as weight scales and blood-pressure cuffs) to track your health between visits.

Connection to community resources and support services.

Ongoing support focused on catching changes early and helping you reduce emergency room visits or hospitalization.

Care that gets you back to living.

Heart failure is a serious condition, but with the right care, many people can improve their symptoms, protect their heart, and live more fully.
